The German hospital landscape is under enormous pressure – economically, in terms of personnel, and in terms of infrastructure. With the Hospital Care Improvement Act (KHVVG), the German federal government passed a far-reaching reform package in April 2025 that marks nothing less than a system change. The aim is to achieve more efficient, quality-oriented, and sustainably financed hospital care.

The Hospital Act opens up opportunities for clinics, operators, and health regions to invest specifically in future-proof structures – supported by billions in funding.

What will change as a result of the KHVVG?


  • Promotion of cross-sector care centers: The law strengthens model facilities that promote outpatient health centers, medical care centers, or specialist clinics—an important step toward reducing overload in emergency rooms and improving patient navigation.
  • Digitalization as the infrastructure of the future: Electronic patient records, standardized documentation and data transmission, and telemedicine, for example, to increase the quality of treatment, improve patient safety, and make work processes more efficient.
  • Introduction of care levels: New care levels for nationwide transparency to improve patient orientation.
  • Flat-rate fees instead of pure case-based remuneration: In the future, hospitals will receive additional remuneration for providing necessary resources, regardless of the actual number of cases. These flat-rate fees are intended to ensure economic stability, particularly in primary care.

50 billion euros for transformation


The KHVVG is accompanied by the new Hospital Transformation Fund (KHTF), which is endowed with 50 billion euros until 2030 and in practice often covers 60-70% of eligible project costs. The specific details of the KHTF are laid down in the KHTF Ordinance. This fund can be used to support hospitals for:
  • Construction and structural adjustments for modern forms of care
  • Digitalization and IT security
  • Energy efficiency and sustainability
  • Development of cross-sector care models

The funding is allocated by the federal states – the key lies in well-prepared, strategically coordinated project applications.
